Overview

Survtapp is a business form and field-collection platform for teams gathering waivers, surveys, checklists or event leads. It suits work that moves between venues or relies on shared tablets, especially where connectivity is unreliable. Its strongest case is combining offline collection with flexible forms and kiosk use; the trade-off is that response, storage and retention caps shape how far the free and entry plans go.

Key features

Forms and offline collection

The builder supports 22 question types, templates, conditional logic, signatures, photos and voice notes. That range can accommodate structured checklists as well as forms that need evidence, while conditional logic can keep follow-up questions relevant to earlier answers. Responses, signatures and photos are queued on the device and sync automatically when a connection returns, a practical safeguard for teams collecting information in the field. It is not a substitute for planning storage and response capacity: those are capped by plan.

Kiosk, integrations and exports

Kiosk mode locks an iPad or Android tablet to one form, making Survtapp a fit for fixed check-in or waiver stations. Assignments and recurring schedules support organized collection work, while exports in CSV, XLSX, JSON and PDF give teams several ways to use survey responses outside the service.

Native integrations include SendGrid, Mailchimp, GoHighLevel, HubSpot and Constant Contact. Webhooks use HMAC-SHA256 signatures; REST API keys are available on paid plans. The meaningful split is that integrations and Open API are included from Business, rather than the lower-cost Free or Solo tiers, so teams that depend on those connections should budget accordingly.

Privacy and security

Survtapp says its database and file storage are hosted in AWS Canada’s Montréal region and that it aligns with PIPEDA and GDPR. Its privacy policy says it does not sell personal information or use advertising or remarketing tools, and describes one-way password hashing alongside technical and organisational security measures. Accounts are for business use, and registrants must be at least 18.

Pricing

Survtapp is freemium. The free plan is free forever and includes 3 users, 100 responses per month, 5 MB of storage, 2 kiosk devices, 3 forms and 120-day data retention. It can suit a small team evaluating a modest workflow, but the storage and retention limits make it a poor fit for sustained evidence-heavy collection.

Solo costs US$19/month or US$190/yr with annual billing. It raises the allowance to 500 responses per month, 2 GB storage, 3 kiosk devices, 5 forms and 12-month retention, while keeping the 3-user limit. Email and knowledge-base support are included. This is the practical step up for a small operation needing longer retention and more capacity without integrations or Open API.

Business costs US$59/month or US$590/yr annually. It allows 5,000 responses per month, 10 GB storage, unlimited users, kiosk devices and forms, and 3-year retention; integrations and Open API, plus priority email support, are included. It fits teams needing shared scale or connected workflows.

Business Plus costs US$129/month or US$1,290/yr annually. It provides 15,000 responses per month, 25 GB storage, unlimited users, kiosk devices and forms, and 7-year retention, with integrations, OpenAPI, priority email support and white-glove onboarding. This is aimed at higher-volume teams with longer record-retention needs or a need for onboarding help.

There are no automatic overage fees, and administrators receive email as they approach plan limits. That avoids surprise overage charges, but approaching a cap still calls for action; response quotas and storage remain finite on every listed tier.

Platforms

Survtapp supports Android, iOS, web and API. Mobile collection is centered on iOS and Android, and kiosk mode is available on iPad and Android. Offline collection and offline signing are relevant advantages for mobile teams, while the API is an option for paid-plan users who need programmatic access.

Who it's for

Survtapp is best suited to businesses collecting signed or evidence-backed forms in the field, at events or through a kiosk, particularly when internet access may drop. It is also a credible option for teams that need scheduled or assigned collection and several export formats. A user seeking a simple waiver-only tool, or a small group that needs high volumes without moving beyond the free caps, may find the plan structure less suitable.

Pros and cons

Pros: Offline queuing covers responses, signatures and photos, so collection can continue through connectivity gaps.
Pros: The 22 question types, conditional logic and evidence fields support more than basic text forms.
Pros: Kiosk mode, exports in four formats and named native integrations give teams multiple ways to collect and move data.
Cons: Free is limited to 100 responses per month, 5 MB and 120-day retention, which constrains ongoing use.
Cons: Solo remains limited to 3 users and 5 forms, and integrations and Open API begin at Business, raising the cost for teams that need those capabilities.
Cons: Higher response allowances, expanded storage and longer retention require progressively more expensive plans.

Alternatives

For a waiver-first workflow, WaiverFile charges no monthly fee on its Booking plan but adds a 6% booking fee rounded up to the nearest whole dollar, with Stripe processing fees separate; it may suit businesses that prefer fees tied to bookings over a monthly subscription. WaiverDrop has a free Starter plan with 10 waivers per month, unlimited templates, document packets, kiosk and touchless mode, and webhooks, making it worth considering when those waiver-specific features matter more than Survtapp’s broader form collection.

WaiverForever has a free Starter plan with 20 submissions per month, basic features and no credit card requirement, a straightforward alternative for very low-volume use. ClickWaiver offers a $29.00 per month Pro plan with unlimited waivers and templates, 3 users, 1 location, white-label, API and webhooks, and an AI Generator allowance of 200 per month; choose it when those waiver-specific capabilities are the priority.

WaiverKit provides a free web-only tier with 1 template, 30 signed waivers per month, branding and PDF export, making it an option for a small web-based waiver workflow. CleverWaiver starts at $9.99 per month for 70 waivers monthly, one template and 20 cents per extra; that may fit businesses whose waiver volume is modest and predictable.

Wherewolf is another freemium alternative, with Launch priced at $89 per month annually or $134 monthly and one waiver type; consider it when a waiver-focused plan is a better fit. Happy Waiver is also a freemium option, with a Cent tier starting at $5/month.

Verdict

Choose Survtapp if your business needs offline mobile collection, signatures or photos, and a path from forms and kiosks to integrations and API access as usage grows. Its combination of offline work and structured form tools is the main reason to choose it; the main reason to look elsewhere is the cost of moving beyond modest response, storage, user and retention limits, particularly when integrations are required.
